What a cleanout is, and why you should care before anything clogs

A cleanout is a fitting with a removable plug — usually a square-nutted brass or plastic cap on a wye — that gives straight-line access into the drain. It is the difference between a drain machine going into the pipe and a drain machine going through your toilet flange after someone pulls the toilet.

That difference shows up on the invoice. Pulling and resetting a toilet to reach a main-line clog adds labor and a new wax ring to the job, and cabling through a roof vent is slower and harder on the cable. A reachable, openable cleanout is the cheap path — which is exactly why code positions them where the problems happen. The rules, verbatim

These are from the residential code as adopted (section numbers move between editions — see the note below):

  • The big one — where house meets sewer (P3005.2.3): “The junction of the building drain and the building sewer shall be served by a cleanout that is located at the junction or within 10 feet (3048 mm) developed length of piping upstream of the junction.” This is the cleanout that matters most, because the line to the street is where roots and grease finally win.
  • Long runs (P3005.2.1): “Horizontal drainage pipes in buildings shall have cleanouts located at intervals of not more than 100 feet (30 480 mm).” A cable has finite reach and loses effectiveness with distance; the spacing guarantees a fresh entry point.
  • Sharp turns (P3005.2.4): “Where a horizontal drainage pipe, a building drain or a building sewer has a change of horizontal direction greater than 45 degrees (0.79 rad), a cleanout shall be installed at the change of direction.” Turns are where cables bind and where solids drop out of the flow — the code puts the access exactly there.
  • Room to actually use it (P3005.2.9): “Cleanouts for 6-inch (153 mm) and smaller piping shall be provided with a clearance of not less than 18 inches (457 mm) from, and perpendicular to, the face of the opening to any obstruction. Cleanouts for 8-inch (203 mm) and larger piping shall be provided with a clearance of not less than 36 inches (914 mm).” A cleanout facing a joist 6 inches away is decorative — a drain machine needs a straight run-up.
  • And visible (P3005.2.10): cleanouts “shall not be installed in concealed locations.” Drywalling over one is the classic remodel sin; an access plate is the fix, not paint.

Where to find yours — a five-minute hunt

  1. Outside first. Walk the line between the house and the street (or septic tank). Look for a capped pipe at grade — often a black or white plug in a small round or rectangular box, frequently near the foundation on the sewer side of the house. Two caps a foot apart is a two-way cleanout: one faces the house, one faces the street.
  2. Basement or crawlspace. Follow the biggest horizontal pipe. Look for the square-nutted plug on a wye fitting, especially at the base of the main stack and wherever the line turns.
  3. Garage and utility rooms, low on the wall where the main passes through.
  4. Older homes, be prepared to lose the hunt. Plenty of pre-war houses genuinely have no exterior cleanout — the requirement post-dates them. If a main-line problem is chronic, adding one is a legitimate, permanent upgrade that pays for itself on the first serious clog: it converts every future “pull the toilet” job into an “open the cap” job. That work is excavation at the line, so it is priced like it — but it is bought once.

Once you know where they are, crack each plug loose once and re-seat it (thread sealant on metal plugs). A cleanout that has been baked shut for thirty years costs its whole advantage at 2 a.m. — the same exercise-it-now logic as your shut-off valves.

Reading the symptoms with cleanouts in mind

  • Multiple fixtures backing up at once → main line. The outside cleanout is where a pro will start — and where you can look first: open it carefully. Standing water in the cleanout means the blockage is downstream (toward the street); a dry cleanout with backed-up fixtures means it’s upstream, in the house. ⚠️ One safety note that belongs in every cleanout article and rarely appears: open a suspected-full cleanout slowly and stand aside — a pressurized column of sewage behind a cap does not wait politely. Loosen the plug a couple of turns, let it weep and equalize, then remove it.

My house has no outside cleanout. Is that legal?

If the house predates the requirement, it is grandfathered — plenty of older homes have none. It becomes a problem the day the main clogs, because access then means pulling a toilet or going through a roof vent. Adding a proper two-way cleanout is a one-time excavation job that permanently converts main-line service from “pull the toilet” pricing to “open the cap” pricing.

Can a cleanout be behind drywall?

Not sealed behind it — the code says cleanouts “shall not be installed in concealed locations.” If a remodel buried one, the fix is an access panel at that spot, not repainting. Concealing it also forfeits the clearance the code requires in front of the opening.

How do I know if a main-line clog is in the house or toward the street?

Open the outside cleanout carefully. Water standing in it means the blockage is downstream, between you and the street or septic — often the utility’s or a sewer contractor’s territory. A dry cleanout while fixtures back up means the problem is upstream, inside the house. That one observation directs the whole repair.
